Instructions:

Forms are to be filled out only by the Varsity Head Coach or Athletic Director.

Please type your data into the PDF, save it, then e-mail it to your AD and to summaries@bval.org.

Please put your school, sport, and gender in the subject line; e.g. San Jose Boys Soccer.

On the signature line of the form, just enter your name. This will act as your signature.

No need to get your Athletic Director or Principal's signature as in the past.

The extra signatures will only be required in the event of a realignment appeal.
